Not all U.S. attempts at quality improvement have been successful. Frequently cited reasons for failure are poor leadership, team-mania (setting up teams before management or employee have been trained in team work), and lack of integration of quality efforts into the whole organization. Obstacles and barriers to success have been researched by Robert J. Masters in an article titled Quality and Total Quality Management. He lists eight common problems that lead to failure. Please explain in one paragraph which two reasons you feel are the most detrimental and explain your reasoning.
